Once we've received your check, we'll be in touch to confirm completion and request 
a mailing address for your certificate. 

Please note: If you will be seeking certification of your participation in this course, you MUST 
be the only individual who completed this training and by asking for course certification thereby 
represent that you completed the work individually.
